Textbook Information for Alt Format
To Request Textbooks in Alternate Format
Students who have print related disabilities, such as low vision, extreme learning disabilities, or students with physical issues and unable to hold a textbook (as supported by submitted documentation), will be approved to have their textbooks provided in alternate format by Student Access Services. Before requesting textbooks the student will have submitted documentation and completed their interactive interview to obtain accommodations.
After approval has been given the student is to provide complete information concerning their textbooks so we can request a PDF file from the publisher. It is a requirement of the publisher that each student provide proof they have purchased the textbook before we are allowed to request the PDF file.
You may either come by the Special Services office, LRC106, to pick up the form, or you may complete the information below. All requested information needs to be provided. Please provide the information for each book needed for the classes you are enrolled in.
Please request textbooks early so you will have them the first day of classes. The beginning of the semester requires often 2 or more weeks to secure needed files.
To help you understand where to find the information being requested:
The Class number and then the Subject & Catalog Number for the course are both listed on your schedule print out.
The book Title & Edition Number, Author, Publisher, Copyright Date and ISBN (13 digit #) are all available at the Rose State College Bookstore.
Finally to verify your purchase of your textbooks, the final field asks that you enter the price you paid for the textbook. Please bring a copy of your receipt with you to Student Access Services when you come to pick up your PDF files.
